What are the causes of toothache? 

     

  • dental caries
  • pulp infections (pulpitis, inflammation of the nerve in the people)
  • periapical infections (dental abscess)
  • dental trauma
  • does not have a dental origin (sinusitis, neuralgia, etc.)

Even if caries and its complications (pulps, abscess, etc.) are the most common cause of toothaches, sometimes they may not be caused by the teeth. For example, pain from a maxillary sinus infection (commonly known as sinusitis) is often felt at the level of the upper teeth.

As a general rule, it is advisable to contact the dentist whenever you report pain in the oral cavity or any other discomfort. 

  • We must call the dentist if:
  • The pain persists for more than 2 days
  • The pain is severe, acute, throbbing
  • The pain is accompanied by edema (swelling) of the region around the tooth
  • The pain is accompanied by fever, altered general condition
  • Pain radiates to neighboring areas (ears, eyes, neck or face)

If you have a toothache, read on to identify the symptoms, causes and treatment options:

1. Dental sensitivity to hot and cold

In the case of dental sensitivity, you will feel slight pain when eating hot or cold food or when breathing cold air, but these pains last only a few moments and then go away.

Causes:

1. thinning of the enamel layer that protects the teeth 

 2. retraction of the gum, revealing sensitive areas on the surface of the root of a tooth (cementum of the tooth)       

3. a small interdental cavity or at the root of the tooth

 4. a filling (plomba) that became furniture. 

In the case of tooth sensitivity, you will feel slight pain when eating hot or cold food, but these pains last only a few moments.

what can you do

Prophylaxis at home Try brushing your teeth with a soft-bristled toothbrush, using a paste with a high fluoride content. There are specific pastes for sensitive teeth (eg Sensodyne). These pastes could reduce sensitivity in areas where the tooth root is exposed.

Therapeutic treatment (obturation)

 Important. If after a few days, after performing the listed dental treatments, the pain persists or increases in intensity, contact your dentist.

2. Pain or discomfort after dental treatments

  Certain dental treatments can produce a slight inflammatory reaction of the dental pulp, which will cause a temporary increase in tooth sensitivity, which should disappear within a few days after the treatment. However, if dental work or more complex endodontic treatments were performed, the teeth may need a longer time to recover. For this reason, patients may feel mild pain in their area for several weeks.

Another reason could be a filling or dental work that was left "too high". In these situations, patients will feel some discomfort and fatigue during mastication or pain in the masticatory muscles or joint.

what can you do

In most cases, the pain will gradually disappear in a few weeks. During this time, you can use mild analgesics (Dexalgin, Algocalmin, etc.).

If the pain persists or increases in intensity, contact the dentist.

If you feel that a dental work or filling is "too high", it is also important to contact your doctor. In these situations, the work will need an occlusal correction. This procedure is absolutely necessary to perform, in order to exclude in the future functional disturbances of the dento-maxillary apparatus

                                3. Sharp pains when you "bite" on that tooth

CAUSE

  • deep decay,
  • Defective filling that has become furniture
  • Fracture of a tooth wall
  • Affecting the dental pulp or the tissue located under the tooth (pulpitis or periodontitis)

The dentist will assess the situation. If the pulp of the tooth is affected, an endodontic treatment is needed. Usually, it is not necessary to extract the tooth, although the proposed treatment may differ from case to case.

A tooth fracture can be difficult to treat. The proposed treatment will depend on the location and depth of the fracture as well as the degree of damage to the pulp of the tooth.

4. Acute pain when eating hot or cold food

If the pains are acute, continuous, i.e. they last even after the stimulus has been removed, it is a sign that the pulp of the tooth (nerve) has been affected.

This disorder is called dental pulpitis, caused by microbes at the level of a deep dental caries that has affected the dental pulp.

what can you do

It is advisable to contact the dentist as soon as possible. Analgesics are mostly ineffective or will only temporarily improve the situation. 

Please note: If it is an acute pulpitis, the pain intensifies and becomes unbearable at night. For these reasons, we actively recommend that you concern yourself with the remedy of dental pain in time, on a daily basis.

Most of the time, the treatment in such clinical situations will consist of endodontic treatment: "removing the nerve" followed by obturation of the root canals. At the end, the tooth will be reconstructed with a filling or a dental crown, depending on the degree of destruction of the dental crown.

5. Constant pain of increased intensity that worsens when touching the tooth

These signs show that the infection has penetrated from the pulp to the tissues located under the tooth (periodontal ligaments, alveolar bone). If the gum is swollen near the tooth, it is a dental abscess.

Sometimes, dental abscesses can evolve without pain, with patients only noticing that the mucosa near the tooth is swollen.

Contact your dentist immediately for relief of pain and discomfort!

The doctor will perform endodontic drainage to remove the pus and may prescribe antibiotic treatment.

To save the tooth, an endodontic treatment is needed, which can extend over several sessions. If the infection is very advanced or the tooth is too destroyed to be reconstructed, the dentist will indicate its extraction. 

6. Constant pain, but it is difficult to specify which tooth is causing the problem

These signs may indicate a dental pulpitis. The "nerve" inside a tooth is infected or possibly even died as a result of the infection.

what can you do

Visit the dentist as soon as possible because, if the condition is not treated, the pain may worsen. You can take sedatives (Nurofen, Algocalmin or others) while you wait to be treated.

After a careful consultation, the doctor will identify the problematic tooth. Correct endodontic treatment will eliminate the pain and save the affected tooth.

7. Pain in the form of pressure at the level of the upper teeth that can also radiate into the jaw

Toothaches can also be caused by other conditions (besides dental ones). Pain caused by diseases of the maxillary sinus can be felt in the teeth and vice versa.

Inflammation of the maxillary sinuses is called maxillary sinusitis. The pain occurs in the cheeks but very often radiates to the upper teeth (molars and premolars).

There are other conditions that can cause pain felt in the teeth and face. For example, bruxism (a condition characterized by excessive and uncontrolled "grinding" of the teeth, especially at night) can cause constant pain and a feeling of pressure in the upper teeth and jaw. Neuralgia

It is absolutely necessary to consult a dentist, but best of all, a maxillofacial surgeon. First of all, it will be established whether the pains have dental origins or other causes.
